Completing Word Applications

Print | Email There are several methods available for submitting
completed applications to Butwin Insurance Group. The applications on this web site have been saved as Microsoft
Word documents. When you open the form (by clicking on its corresponding hyperlink on the
Applications page), it opens in a new Internet Explorer window. Simply fill in
each field and press TAB to move to the next one. When you are finished, we recommend that you save a copy
of the completed application on your computer. Saving the Completed
Application When you have finished filling out the application, you can save it on your computer by
following these steps:
- Click on File > Save As.
- In the Save As window,
- select your My Documents folder from the Save in field at the top of the screen
- type a descriptive name for your completed application in the File name field at the bottom of the screen
- in the Save as type field, be sure that Word Document (*.doc) is selected
- Click on the Save button
- Click on File > Close to close your Internet Explorer window.
- If prompted to save changes, click on No. (You have already saved the form to your computer, so
there's no need to do anything further on this window.)
Printing the Completed Application When you have
finished filling out the application, you can print a copy by following these steps:
- Click on File > Print
- When the Print window appears, click on OK

Note: You can also click on the printer icon on your browser toolbar. |

- You can now fax your printed application to Butwin Insurance at 516-466-4213. No cover letter is necessary,
provided that you have accurately completed all of the contact information on the application.
