How do I obtain a Certificate of Insurance?
Click here to request a certificate.  Complete the form in full, then fax or email it to our office.  It will be emailed directly to our Certificate Department. Please be sure to know in advance if the Certificate Holder is requesting to be included as Additional Insured or just a Holder.

How do I bind a new business account?
Please take a few moments to read over the cover page on our proposals. This gives detail as to what needs to be done to bind an account. Different carriers have different requirements. They can be different, so again please read our Proposal Cover Page.
Why are binders only for 30 days?

Binders are only a temporary form of insurance until the actual policy is issued; within those 30 days you will receive the actual policy. If proof of coverage is needed, you should request a Certificate of Insurance.
How do I report a claim?
 
Are agents allowed to alter Certificates issued by BUTWIN?
 
Should I send in the Declaration pages of the prior policy?
Yes, this is most helpful to our office to assist in securing an apples to apples quote for your prospect. This also helps us obtain a better rate by knowing they do have prior coverage in force. It helps us do a better job for you with the prospect.
When do I mail the check?
Upon receipt of the binder, please immediately mail the completed original paperwork and check to Butwin Insurance Group at 60 Cutter Mill Road #414, Great Neck, NY 11021.
How do I obtain a book of business report?
Please simply send an email to allstate@butwin.com and title your email “Book of Business Request”.  This will immediately be forwarded to our bookkeeping department to comply with your request.
What is the amount of commission to the producer and when/how do we get paid?
Our producers are compensated 40% of the commission we receive from our carriers on all lines of business excluding Commercial Auto which we pay 50%. The percentages of commission Butwin receives from our carriers vary by carrier and line of business. Butwin issues a paper check to your office along with a Producer Statement that will show what you are being compensated for. These checks are issued at the end of each month and your office should receive them by the 15th of the following month.
Does BUTWIN accept credit card payments?
Only Utica First and Progressive will accept credit card payment for deposits. All other carriers require a check. After the deposit is received and the policy is issued, the insured can contact their service center to set up Electronic Funds transfer if they desire. Please keep in mind this helps with retention.
